Birthday Book Club Registration

Dear Parent: We invite you to commemorate your child’s birthday by putting a brand new book in your child’s library! As a participant of the Birthday Book Club your child will: -choose a new book from the library’s Birthday Bookshelf -have a permanent bookplate with their name placed in the inside cover -receive a certificate & bookmark, presented during their library period -have the first opportunity to check out the new library book (Please remind your child that this book will belong to the school library) To enroll: Return the bottom portion of this form to your child’s teacher with a check for $15.00 payable to Hudson PTO. Please mark the envelope ‘PTO Birthday Book Club Attn: Stephanie Bielecki.’ Enrollment forms are accepted at any time during the school year through April 30th regardless of birthday month.
